Placing a bridesmaids order through Hitched Maids is easy. You will find the 3 forms below that are necessary to move forward with your order. Please download and complete each form and email it to info@hitchedmaids.com or fax it to 202-315-3502.

(1) Bridal Party Info Form for Brides (PDF) Hitched Maids only needs one Bridal Party Info Form to be completed per order. It can be submitted by the bride or the bridesmaid coordinating the order.

(2) Bridesmaids Order Form/Policy Form (PDF) One form must be submitted for each bridesmaid.

(3) Credit Card Authorization Form (PDF) This form is used to pay for bridesmaids dresses via credit card. Dresses can also be purchased with a check payable to “Hitched”.
